About the Role
* The Cancer Care Coordinator will be responsible for working as part of the Oncology team in Wagga Wagga, Griffith, or Young.
This position involves collaborating with various services including Local Health District Health Services, the Riverina Cancer Care Centre, tertiary referral cancer centers in Canberra, Sydney, and Melbourne, Community Health, GPs, Medical Specialists, and other cancer service providers to enhance access and ensure the effective coordination of care.
The treatment pathway for a person diagnosed with cancer is often complex, and the Cancer Care Coordinator has an opportunity to improve the quality of treatment and life for the cancer patient.
Funding for this position is provided by the Cancer Institute NSW to establish clear referral pathways with local GPs and service providers for cancer, assisting patients in accessing cancer treatment and improving continuity of care within and external to the Local Health District.
Key Responsibilities
* To act as a professional role model delivering high-level capacity for patient advocacy while demonstrating the ability to provide effective clinical leadership and practice as an autonomous professional within a multidisciplinary team environment.
Requirements
* Clinical Nurse Specialist Grade 2 (CNS2) and Clinical Midwife Specialist Grade 2 (CMS2) are crucial members of the Murrumbidgee Local Health District Nursing and Midwifery Workforce.
* The CNS2 provides high-level patient-centered, evidence-based clinical nursing care and is expected to embrace and lead a practice development philosophy in the delivery of that care.
* The CNS2 encompasses core criteria of CNS1 with additional characteristics, exercising extended autonomy of decision-making and exercising professional knowledge and judgment in providing complex care requiring advanced clinical skills.
