**Job Description:**
As a Care Coordinator, you will be responsible for leading the delivery of high-quality clinical care to residents with complex needs. This role requires strong leadership and workforce management skills, as well as the ability to coordinate clinical documentation and compliance reporting.
Key Responsibilities:
* Oversee resident assessments and develop tailored care plans
* Coordinate clinical documentation and compliance reporting (AN-ACC)
* Lead and mentor a team of RNs, care staff, and volunteers
* Collaborate with external providers to ensure seamless care delivery
* Monitor and improve clinical programs and quality indicators
* Champion a culture of safety, wellbeing, and continuous improvement
Requirements:
* AHPRA Registration as a Registered Nurse (RN) with a current Practicing Certificate
* Minimum 3 years' post-graduate experience in aged care or acute clinical settings
* Proven leadership and workforce management skills
* Strong clinical assessment, care planning, and documentation capabilities
* Intermediate computer literacy (Word, Excel, Outlook, PowerPoint)
* A commitment to quality, safety, and continuous improvement
* Current National Police Check – or willingness to acquire
* Current First Aid & CPR certification – or willingness to acquire
* A Valid driver's licence
