About the Opportunity
The Clinical Nurse Consultant (CNC) –Community Aged Care, provides expert clinical leadership and consultancy, throughout Murrumbidgee Local Health District (MLHD). Working as part of the Community Aged Care Team, this district‐wide role supports older people with complex health needs to remain safe and independent at home. The CNC collaborates with multidisciplinary teams, carers and external services to deliver high‐quality, evidence‐based, person‐centred care and lead practice development across the service.
As the CNC –Community Aged Care, you will be responsible for:
* Providing expert clinical consultancy and advanced care to older people with complex and chronic needs
* Developing, implementing and evaluating coordinated care plans with patients, carers and multidisciplinary teams
* Leading and role‐modelling evidence‐based practice and ongoing clinical service development in community aged care
* Delivering specialist education and advice to clients, carers and health professionals on complex aged care issues
* Leading and contributing to quality improvement, clinical audit, research and service development initiatives
* Implementing innovative models of care, including new treatments, technologies and therapeutic approaches
* Contributing to strategic and operational planning, service evaluation and future direction of Community Aged Care

About You
Our ideal candidate will demonstrate:
* A Minimum 5 years full‐time equivalent post‐registration experience, with relevant post‐registration qualifications in aged care, community health or a related specialty.
* A demonstrated expertise in community‐based aged care and the management of complex clinical presentations.
* A current Authority to Practice as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
* A strong understanding of the five Clinical Nurse Consultant domains.
* Excellent communication, leadership and consultancy skills.
* Commitment to continuous learning, research and service improvement.
