Role Overview
The Adult Community Mental Health Service North is a case management service that provides expert mental health care, including assertive case management, triage, holistic physical and mental health assessment, clinical risk management, and crisis intervention within the community setting.
As a Nurse Practitioner, you will provide specialist care to consumers and their families within their designated region, identified as at risk of or having moderate to severe mental health disorders, as part of a multidisciplinary mental health team.
* Provide specialist mental health assessment and clinical management to consumers of Adult Mental Health Services to promote positive mental health outcomes and hospital avoidance.
* Work in accordance with shared care principles and within a multidisciplinary treatment team to assist consumers to access services and enhance their engagement with internal and external agencies.
* Take a lead role in the development and implementation of system changes and service improvements in Adult Mental Health Services.
* Be responsible as a leader, advisor, and mentor for other nurses and clinicians to ensure high-level clinical excellence within Adult Mental Health Services.
* Contribute to building collaborative working relationships and positive team culture across all services in AMHS, including community services and inpatient units.
Key Responsibilities
1. Develop and implement effective case management plans to meet the needs of consumers and their families.
2. Provide education and support to consumers and their carers on mental health issues and recovery processes.
3. Collaborate with other healthcare professionals to ensure comprehensive and coordinated care for consumers.
4. Identify and address potential risks and challenges to consumer safety and well-being.
5. Contribute to the development and evaluation of policies and procedures that promote best practice in mental health care.
Selection Criteria
1. Registered with the Nursing and Midwifery Board of Australia and endorsed to practice as a Nurse Practitioner.
2. Completed a specialist tertiary graduate or postgraduate mental health/psychiatric nursing qualification; or completed, prior to the transfer of nurse education to the tertiary sector, an accredited hospital-based program that leads to registration as a psychiatric nurse by the relevant nurse regulatory authority within Australia, New Zealand, Canada, or the United Kingdom.
3. Current Working with Vulnerable People Registration.
4. Current Driver's Licence.
