About the Role As a Case Manager – Next Step Plus, you will support young people aged 15+ as they transition from Out of Home Care to independent adult living.
You will work alongside young people to build life skills, confidence and connections, supporting access to funding, housing, education, employment and community supports.
The role also supports young adults who have exited care up to the age of 25 and contributes to the delivery of Extended Post Care Support funding .
This role is permanent full-time, based in Toowoomba .
Key Responsibilities Provide trauma-informed, person-centred case management to young people impacted by complex trauma.
Apply Therapeutic Crisis Intervention (TCI) to support behaviour regulation and safety.
Develop and review transition plans supporting independent living and community engagement.
Work collaboratively with Child Safety to support transition and financial planning for young people aged 15+.
Support peer connection, mentoring and social inclusion.
Build strong relationships with families, community services and government stakeholders.
Embed culturally safe practice and support young people to maintain culture and identity.
What You'll Bring Tertiary qualification in Social Work, Psychology, Human Services or a related field (or equivalent experience).
Current Queensland Driver's Licence.
Experience working with young people with complex needs.
Strong knowledge of issues impacting young people transitioning from care.
Ability to manage competing priorities and maintain quality documentation.
Strong communication and relationship-building skills.
Successful candidates will be required to clear probity checks including National Criminal History Record Check and Working with Children Check (where relevant to the role).
