About the Service

The Country Wellness Connections (CWC) program provides psychosocial support services and community connection to people experiencing mental ill health in the Riverland region of South Australia. The CWC service delivers a person‐centered approach to recovery and a range of non‐clinical community‐based supports tailored to meet the needs of clients to help them get on with their lives.

About the Role

We are seeking a recovery‐orientated and goal‐focused Community Mental Health Practitioner for a fixed term, fulltime (38 hours per week) opportunity until 30 June 2026 in the Riverland area. You will play an integral role in working collaboratively with clients to achieve their recovery goals through one‐to‐one support, shared supports, workshops and group work facilitation to enhance recovery, personal growth and activities of daily living.

Key responsibilities

* Plan and develop group work programs that will assist clients to build their skills, focus on their recovery and work towards transitioning back to their natural community.
* Undertake necessary assessments in line with the clients funding need and choices.
* Make linkages, build relationships and referral pathways with service providers, clinical partners, government agencies and healthcare providers to facilitate clients living the life of their choosing within the community.
* Proactively manage a growing client base ensuring all agreed support coordination hours are delivered in a way that provides most value to the client.

What you'll bring

* Tertiary qualifications (minimum Certificate IV) in Mental Health, Peer Work, Psychology, Social Work, Occupational Therapy or other health related field asailymighted by Mind.
* Current valid drivers licence and willingness to travel across a designated region to fulfil the duties of the position.
* Proven experience in Community Services, Mental Health, Disability, Social Welfare, Housing or Healthcare or Government sectors is required.
* Experience and expertise in working directly with people with mental health issues, complex needs and with their families and carers.
* High level organisational skills with demonstrated ability to prioritise, manage multiple complex tasks concurrently, work under pressure and meet deadlines.
