Aboriginal Health Case Manager
We are seeking a highly skilled and compassionate Aboriginal Health Case Manager to join our team. This vital role involves supporting individuals experiencing severe and complex mental health challenges through person-centred case management and care planning.
* Conduct thorough clinical assessments and develop tailored care plans that address the unique needs of each individual;
* Establish and maintain strong relationships with GPs, psychiatrists, psychologists, and allied health professionals to ensure seamless coordination of care;
* Provide ongoing support and advocacy for clients during medical and social appointments, ensuring their voice is heard and their rights are protected;
* Engage with family members and carers where appropriate, fostering a sense of community and connection;
* Maintain accurate and comprehensive documentation, participating in regular team meetings to share knowledge and best practices;
* Contribute to the development of health promotion initiatives, community engagement strategies, and recovery-oriented care approaches.

Requirements
* Degree in Social Work or a related field;
* Minimum 2-5 years' experience in community mental health or social support roles;
* Strong understanding of psychosocial support principles and trauma-informed care practices;
* Excellent communication and interpersonal skills, with the ability to engage stakeholders effectively;
* Current WWVP check and valid driver's licence required;
* Australian work rights essential.
