About The Role
An exciting opportunity exists to join the Western Virtual Patient Flow Access & Command Centre.
Delivering services across multiple locations, the Clinical Nurse Specialist will work in partnership with Patient Flow, Inpatient, and Connected Care in Community teams to support effective discharge coordination processes and achieve key performance outcomes.
This role operates on a rotating roster across morning and afternoon shifts, seven days a week, and supports a coordinated, multidisciplinary team approach to optimise patient flow and continuity of care.
