Support Clinician Role
We are seeking a motivated clinician to provide comprehensive support to individuals with dementia and their families.
* This role involves providing vital support through home visits and clinics for dementia and aged care;
* Working closely with a multidisciplinary team to develop and implement individualised care plans;
* Connecting with patients and families from diverse cultural backgrounds, demonstrating empathy and understanding;
* Collaborating with a Rehabilitation Specialist to ensure seamless care delivery;

Your Skills and Experience
To be successful in this role, you will possess:
* A current registration with AHPRA as a Registered Nurse;
* Proven clinical experience in dementia, delirium, or aged care case management;
* Excellent communication, assessment, and time management skills;
* A strong work ethic and ability to work autonomously and within a team;
* Cultural awareness and empathy to support diverse populations.
