Aboriginal Health Practitioner – Diabetes Education

About the role

The Aboriginal Health Practitioner (AHP) in Diabetes Education is a varied role, working closely with another AHP out in the community at various facilities, as well as within the Diabetes Unit, educating patients and families living with diabetes. You will also be responsible for co-ordinating the Wellness Program and the Elders Exercise Group at cardiac rehabilitation twice weekly.

You will work alongside a dedicated multidisciplinary team of Allied Health Professionals including Diabetes Educators, Dietitians, Administration staff, Endocrinologists and Paediatricians.

Western NSW Local Health District (WNSWLHD) serves 278,000 people including over 36,000 Aboriginal people - 13% of the District's population - the highest proportion of Aboriginal people in any NSW LHD.
