About the role:

Ballarat Community Health is dedicated to providinghigh-quality healthcare services to our community. We believe in aperson-centered approach, ensuring seamless coordination of care across alllevels of our organisation.

Supporting the region's ageing population withwell-coordinated, tailored health services in and close to home is a strategicpriority within our 2024-2029 Strategic Plan.

The Care Partner (Clinical) will carry a reduced direct caseload of care participants whilst also providing clinical oversight and input tocare planning across the HCP Service.

* Providingcomprehensive care planning, brokering services, management of budgets,ensuring that care aligns with participant needs and preferences forparticipants assigned as care partner.
* Providingclinical input to Care Plan development and review, conducting clinicalassessments using standardised tools and contribute clinical expertise tomultidisciplinary care planning and review
* Identifying,sourcing and maintaining and supporting the team in applying appropriateclinical protocols.
* Partakein an on call roster, where service delivery is provided between the hoursof 5.30 pm - 9pm Monday through Friday and 10 am to 4 pm Saturday andSunday and Public holidays.

What you will bring to the team:

* Qualificationas a Registered as a Division 1 or Division 2 Nurse under the AustralianHealth Practitioner Regulation Agency (Nursing and Midwifery Board ofAustralia) or Member of Australian College of Mental Health Nurses
* Extensiveexperience as a Care Manager, with in-depth knowledge and understanding ofthe Aged Care Quality Standards.
