In addition to base salary, all roles attract 12% Superannuation + Salary Packaging

Join South Western Sydney Local Health District and play a pivotal role in improving access to high‑quality, person‑centred healthcare for people with intellectual disability. As the Intake Officer for the Specialist Intellectual Disability Health Team (SIDHT), you'll help drive meaningful change by coordinating and completing the multidisciplinary clinical intake process. Your work will ensure referrals are appropriate, comprehensive, and aligned with the SIDHT model of care, ultimately supporting individuals, families, and carers to receive timely and effective specialist health services. This is an opportunity to contribute to a collaborative, multidisciplinary team that is dedicated to delivering compassionate, equitable, and impactful healthcare to a vulnerable population.

We are looking for someone who brings not only clinical capability, but also compassion, adaptability, and a genuine commitment to making a difference in the lives of people with intellectual disability and their families. You may be the right fit if you are a Registered Nurse or Social Worker with experience in clinical or psychosocial assessment, triage, and working within healthcare settings. You communicate with warmth, clarity, and respect, and you value partnership‑based, person‑centred care. You're comfortable navigating a busy environment, able to prioritise and manage multiple tasks with accuracy and professionalism, and are confident using electronic medical records, patient administration systems, and Microsoft applications. If you can work independently while also contributing meaningfully within a multidisciplinary team, this role may be the perfect next step for you.

Selection Criteria
To help us assess your suitability for the role, please tell us as much as you can about yourself in response to the questions below.

* Current nursing registration with the Nursing and Midwifery Board of Australia (AHPRA) or a Bachelor degree in Social Work or equivalent qualification that provides eligibility for full membership to the Australian Association of Social Work.
* Recent experience in clinical and/or psychosocial assessment of clients in health care settings.
* Highly developed interpersonal skills including the demonstrated ability to engage with clients fromculturally and linguistically diverse backgrounds and highly developed customer service skills.
* Demonstrated experience and knowledge in using patient administration systems or electronicappointment scheduling systems and other computer applications e.g. Microsoft Word, Excel.
* Proven ability to work independently and as a part of a team to organize, prioritize and meet deadlines ina busy environment.
* Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
