**About the Role**:
As a **Home Care Coordinator**, you will be dedicated to working with clients living in their own homes and community to live their best lives. You will ensure effective and efficient delivery of flexible, individually tailored Home Care packages to people who fall within the eligibility criteria of the Home Care Package Program and ensure compliance with the Aged Care Quality Standards.
To be successful in this role you will have:
- Excellent communication skills both written and verbal.
- Compassion, and empathy for others, with a natural ability to adapt to the client’s style.
- Able to prioritise work and enjoy working in a dedicated team environment that is often changing to meet our client’s needs.
- Excellent customer service.
- Proficient IT Skills using Microsoft 365 suite, and other databases.
Essentials:
- Covid-19 and Flu 2024 Vaccination and ongoing boosters are a requirement.
- National Police Clearance (No more than 2 years old)
- Unrestricted SA Driver's License
Qualifications / Experience:
- Certificate IV or higher in Individual Support or Aged Care or equivalent
- Experience in Home Care Case Management (preferred)
- At least 1 year of experience in similar roles
- Experience of the My Aged Care processes, and home care funding streams for home care packages
- Knowledge of the Aged Care Quality Standards
- Experience in understanding case management practices
- Experience in managing Individual budgets and support plans
