Care Coordinator, Residential In-Reach

Continuing & Complex Care

Permanent part time, up to 72 hours per fortnight available

Nurses & Midwives Enterprise Agreement – Clinical Nurse Consultant A ($59.79 p/h)

We have up to 72 hours per fortnight available to be offered on a permanent part time basis. RIR works across 7 days per week and you must be willing to work a rotating roster.

Our preferred candidates will have:

* Degree / Masters in Nursing / Gerontology

* Current registration with the Australian Health Practitioner Regulation Agency (AHPRA)

* A high level of Clinical and practical clinical knowledge

* Ability to provide a high standard assessments, treatment and intervention to reduce the number of presentations from Residential Aged Care Facilities clients presenting to the Emergency Department.

* Ability to work effectively with other Health Care Professionals across Aged Care, AV, & VVED

* Commitment to providing a customer focused service

* Capacity and willingness to be flexible and adaptable

* Excellent organisational and time management skills

* Excellent oral and written communication skills

* Ability to work independently and to take initiative

* Excellent problem-solving skills

* Computer Skills in Microsoft Office, particularly Word & Excel.

IMPORTANT: Some roles in the healthcare sector are required to be vaccinated annually for Influenza (unless a valid exemption applies). Evidence of vaccination (or exemption) may be required prior to commencement.

All appointments with the West Gippsland Healthcare Group are subject to the satisfactory return of professional reference checks, a NDIS worker screening clearance and a current Employee Working with Children Check. All employees must hold current entitlement to work within Australia. A six month probationary period applies to all new appointments.
