Achieve Something Great
In this role, you will make a meaningful contribution to mental health recovery by delivering high quality, evidence based and strengths focused care within a multidisciplinary team across either the Adult Care Coordination Team (ACCT) or the Assertive Outreach Team (AOT).

ACCT provides community care coordination for adult consumers with complex mental health needs that cannot be met by primary level services. This involves a range of consumers both voluntary and involuntary on Community Treatment Orders (CTO). ACCT operates Monday – Friday 8:30am – 5pm.

AOT is an assertive care coordination team for consumers with highly complex mental health, substance misuse, challenging behaviours, and forensic needs. This involves a range of consumers both voluntary and involuntary on Community Treatment Orders (CTO). AOT operates on a rotating roster Monday – Sundays, shifts are 8:30-5pm or 2-10:30pm weekdays and 10 – 6:30pm on weekends.

You will undertake comprehensive mental health assessments, develop, implement, and review individual recovery plans, provide therapeutic interventions, and coordinate referrals and discharge planning. Working collaboratively with consumers, their families and natural supports, you will provide education, skills development and advocacy, while actively liaising with general practitioners, community agencies, NGOs and other mental health services. You will also support service excellence through accurate and timely clinical documentation using the electronic Medical Record (eMR), participation in quality improvement and service development activities, and by helping to build strong partnerships across the health and community sectors.

As a pool clinician, you'll begin by supporting one of our teams to fill a current vacancy. As new opportunities arise across our mental health services, you may be offered the chance to transition to other teams—providing you with valuable experience and exposure across the diverse services within SWSLHD Community Mental Health. Future roles may become available at Ingleburn, Bankstown, Campbelltown, Ambarvale, Liverpool, Fairfield, and Wollondilly.
