Job Title

We are seeking a full-time leader to support individuals with daily living and defined programs.

This role is perfect for someone looking to develop their career in the industry and make an impact.


Key Responsibilities

* Lead a team of professionals to deliver supports in the home or community using the active support model.
* Negotiate service booking agreements, effective support planning, and implementing supported decision making to ensure clients meet their individual care and support needs.
* Ensure all clients have current and up-to-date documentation, including client files, support plans, funding agreements, risk assessments, and health plans.
* Assist in identifying client needs by engaging in a strengths-based approach to support planning.
* Provide support to clients using the active support model, giving them as much control and independence as possible without compromising duty of care.
* Ensure clients are supported to make choices on their day-to-day needs, goals, and dreams.
* Document and staff training on positive behavior support.
* Encourage clients to undertake regular activities of their choice in the community consistent with their NDIS plan.
* Develop skills for independent living consistent with the NDIS plan.
* Support contact and communication between clients and their families and friends.
* Liaise with other service providers, family, and advocates.
* Support clients to participate in their NDIS planning processes and provide evidence where required.


Staff Management

* Develop and maintain a positive staff culture that supports the organization's philosophy, legislation, and policies.
* Provide supervision and role modeling to ensure staff work cooperatively and effectively as a team.
* Provide client roster needs to the Rostering Team.
* Ensure all staff adhere to policies and procedures.
* Conduct monthly team meetings and staff development, including medication competency and induction to client supports.
* In consultation with the Services Manager, complete informal staff discussion/counseling.
* Conduct Annual Professional Development Reviews.


Administration

* Establish a thorough working knowledge of relevant policies and procedures and work consistently with these at all times.
* Record and capture all requested data.
* Ensure Environmental Risk Assessments have been completed annually for supports delivered in client homes and comply with legislative and funding body standards and organizational Occupational Health and Safety standards.
* Manage individual client budgets and maintain records.
* Complete reports in a time-efficient manner.


Requirements

* Previous experience working in the disability or aged care field.
* Understanding of current disability services legislation and standards.
* Experience in managing staff.
* Ability to work in a team environment and maintain good working relationships, and the ability to work independently.
* Self-motivation and ability to think and plan laterally.
* High level of computer literacy.
* Effective time management skills and the ability to prioritize workload, set goals, and meet deadlines.
* Excellent written and verbal communication skills, negotiation, and liaison skills.
* Understanding of positive behavior management.
* Respect for cultural and religious needs of clients.
* Tertiary qualifications in Community Services or equivalent, preferably at Diploma level.
* Strong commitment to CLRS vision and values.


Additional Requirements

* Level Two First Aid.
* Evidence of three doses of COVID-19 vaccination or medical exemption.
* Working with Children Card.
* NDIS Worker Screening Check.
