Job responsibilities include:
1. Assisting NDIS participants and their families to identify needs through assessment and planning.
2. Developing, evaluating, and delivering evidence-based support plans within complex support environments.
3. Creating meaningful goals and practical strategies using a positive behaviour support framework.
4. Ensuring plans reflect participants' goals, passions, and strengths.
5. Building capacity and strengths of those supporting participants.
6. Collaborating with a multidisciplinary team, including mentoring less experienced practitioners.
About You
* Proficiency or higher as a Behaviour Support Practitioner with evidence of qualification.
* Significant experience supporting individuals with behaviours of concern.
* Excellent report writing, communication, and interpersonal skills.
* Valid driver's license and reliable vehicle.
* Genuine passion for the disability sector.
