About the Role
We are seeking a compassionate and skilled Community Palliative Care Registered Nurse to join our supportive team. In this rewarding role, you will provide high-quality, person-centred care to clients in their homes, supporting not only their comfort and dignity but also providing guidance and reassurance to families and carers during a significant stage of life.
Working collaboratively within a multidisciplinary team, you will deliver specialist palliative care across the client journey. This includes comprehensive assessment, care planning, symptom and pain management, and ongoing nursing care. You will use your expertise to coordinate services and ensure clients and their families receive the best possible support.
This position operates on a rotating 7-day roster, with shifts available across days, evenings, and nights—offering variety, flexibility, autonomy, and the chance to make a meaningful impact every day
Key Responsibilities
· Provide holistic, evidence-based palliative nursing care in the community setting.
· Undertake thorough assessments and develop individualised care plans.
· Deliver effective pain and symptom management strategies.
· Coordinate care in partnership with families, carers, GPs, and other community services.
· Support and educate families and caregivers, helping them feel confident and informed.
· Contribute to the ongoing development of palliative care services within the team.
What You'll Bring
· Current Registered Nurse (RN) qualification with AHPRA registration.
· Additional qualifications and/or strong experience in palliative care.
· Demonstrated experience in community-based nursing.
· Advanced assessment, care planning, and case coordination skills.
· Ability to work both independently and as part of a multidisciplinary team.
· Strong communication, interpersonal, and negotiation skills.
· A genuine passion for providing compassionate, person-centred care.
Essential criteria:
· The legal right to work in Australia.
· A valid driver's license and a reliable vehicle
· National Police Check
· Working with children check
· Certificate in First aid & CPR (current)
· NDIS Worker Screening Check
· Two professional references
