Responsibilities
* Lead clinical social work service provision, including optimisation of client care delivery and mentoring/development of staff.
* Maintain a case load with case management for multiple clients from admission to discharge.
* Conduct comprehensive psycho-social assessments, care planning, risk assessment, education, advocacy, counselling, liaison with community agencies and relevant parties including medical teams.
* Engage with clients' families and/or carers as part of the support plan.
* Work autonomously in the community setting when completing in-home assessments and interventions.
* Collaborate as part of a multidisciplinary team to deliver low-intensity, home-based therapy programs for frail and socially complex older adults ready for discharge.
Qualifications
* Social Work degree and eligibility for the Australian Association of Social Workers (AASW).
* Aged Care Check.
* Valid NSW Driver's Licence.
* Applicable vaccination requirements for a Category A role.
