Rose-Mary is seeking a part-time (20 hours per week) or Full-time (40 hours per week) LPN to provide health care to individuals in Intermediate Care Facilities (ICF). The primary role of the LPN is to assure the appropriate medical care is being provided and that all information relative to the well being of the individuals is communicated to the care team.
POSITION RESPONSIBILITIES:
* Work flexible schedule to meet the needs of the individuals. Assume part of the on-call rotation with the other nursing team members.
* Observe and interact with the individuals to learn and understand each individual's behavior and needs.Records all observations and care in adequate detail on the Nursing Notes.
* Follow all physician's orders for the care of each individual i.e., treatments, medications, etc. and provides appropriate documentation.
* Maintain nursing records on all individual's medical conditions and test results in accordance with established protocol and procedures.
* Ensure compliance with all Medicaid requirements relating to nursing.
* Ensure privacy and confidentiality in record keeping and other matters relative to the individual's care including diagnoses, prognosis, and medications.Imparts information to staff strictly on a need to know basis.
* Orient staff to symptoms and prognosis of specific diseases and syndromes.
* Provide in-service training as needed.
* Assist House Manager and QIDP in keeping parents and staff informed of individual's medical conditions, needs, progress, etc.
* Participate in IDT meetings, conferences, and other staff meetings as needed.
* Order medication and medical supplies as needed.
* Schedule Individuals medical appointments and attends such appointments with individuals when appropriate.
* In cooperation with House Manager and QIDP, maintain an atmosphere and environment that is conducive to both the physical and emotional well being of the individuals.
* Communicate with the Director of Nursing, House Manager, QIDP, and Supervisor on specific and general individual/program related problems or issues of concern.
* Follow all policies and procedures of the Home and conducts self in a professional manner as a model for the DSP Staff.
* Maintain Delegations of Nursing statements
* Complete Quality Assurance assessments on a regular basis to ensure compliance of rules and regulations
* Maintain Medical Appointment Tracker on all individuals to ensure they are up to date on immunizations, lab work, TB tests, and appointments.
* Follow the Telephone Triage Protocols for Nurses by Director of Nursing.
* Perform other duties as assigned.

UNUSUAL WORKING CONDITIONS:
* Work will include evenings and weekends
* Assumes on-call responsibilities with nursing rotation
* May be exposed to contagious diseases
* May be exposed to aggressive or otherwise challenging individuals
MINIMUM QUALIFICATIONS:
The professional nurse shall have the following qualifications:
* Graduate of an accredited School of Nursing.
* Licensed by The State of Ohio with appropriate certification to pass medications.
* Minimum of one-year experience in Medical/Surgical Nursing and/or working within a facility for the Developmentally Disabled.
* Valid Ohio driver's license with less than 4 points and be insurable under the company policy.
