About The Position
The Housing and Accommodation Support Initiative (HASI) supports people with a mental health condition to build independence and participate in their community. The Referral Pathway Coordinator's primary purpose is to promote and manage HASI program's referral pathway to ensure equitable access. Responsibilities include triaging referrals, conducting assessments, providing support services, screening referrals, making first contact calls, managing a small caseload, reviewing functions with the Service Manager, and monitoring referral pathway KPIs and data collection. This role is based across Ulladulla and Nowra offices, full-time, fixed-term until 30 June ****, with a rostered day off each month.
We Are Looking For Someone With
Current AHPRA registration as a psychologist or provisional psychologist, mental health nurse, occupational therapist (mental health), or accreditation with AASW as a mental health social worker.
Demonstrated skills and experience in a direct service provision role within a psychosocial support program or mental health service.
Understanding of the operations of public mental health services and relevant legislative frameworks, including the National Framework for Recovery Oriented Mental Health Services.
Experience in developing, refining, and implementing processes and procedures with attention to detail.
