Community Health Care Professional
We are seeking a highly skilled and dedicated Community Health Care Professional to provide direct nursing care and assessment to clients in their homes and nominated clinics.
* Implement comprehensive care plans, coordinate and plan ongoing nursing care to ensure optimal health outcomes for clients.
* Provide high-quality nursing services, including assessments, interventions, and education to promote client independence and well-being.
* A tertiary qualification in Nursing.
* Current AHPRA Registration as Division 1 Nurse.
* Completed a graduate year or minimum two years' post-graduate experience in community or four years acute nursing.
* A current Working with Children Check.
* A current Victorian Drivers Licence.
