**About the Role**

The introduction of the National Disability Insurance Scheme (NDIS) marks a significant change in how disability services are funded and how service in provided to the NDIS participant.

The Support Coordinator is responsible for connecting people with a National Disability Insurance Scheme (NDIS) Plan to access a range of supports which align with the support items in their NDIS plan and that suit their individual needs, across one or more providers.

As a Support Coordinator, you will provide varying levels of coordination and transition support for ages 0 to 65 years old. As well as considering individual's circumstances and level of complexity. Participants will be matched with community based and/or mainstream programs to fulfil their individual needs whilst receiving continuous support and assistance. It is essential you have experience in this sector to provide the support necessary.

The Support Coordinator's duties may vary depending on the specific needs of the customer and the goals outlined in their NDIS plan. Common day to day duties of position may include (but not limited to) assisting the participant to:

- To work under general direction from senior manager however able to exercise initiative and judgment where practices and direction are not clearly defined by the NDIS or Harmony Place Practice Manual.
- Present possible support options available to the participants allowing them to exercise choice and control.
- Negotiate services to be provided and their prices
- Develop service agreements and create service bookings with preferred providers
- Arrange for any assessments required to determine the nature and type of funding required
- Determine the budget for each support type and advise any relevant plan manager of the breakdown of funds
- Liaise with any plan manager to establish the appropriate claim categories and attribute the correct amount of funds
- Liaise with NDIS support workers at times to provide direction of tasks that need to be completed on shift to support in achieving an individual goal or health outcome
- Support the participant to link with mainstream or community services (i.e. housing, education, transport, health) along with hosting stakeholder meetings with external service providers to coordinate the most suitable care for the participants.
- To assist in the development action plans in the coordination of the supports. This can include developing a set priorities, plan and organise supports to best meet the participants needs.
- Assess and manage risk in line with organisational policies and procedures.

**About You**
- Passionate about the NDIS sector and assisting clients to access services suited to their needs
- Farsi, Dari or Hazaragi speaking would be an advantage.
- A diploma or tertiary qualification or degree in Human Services, Social Work or related disciplines
- Demonstrated strong time management skills including prioritization of competing tasks
- Demonstrate understanding of the challenges faced by people with a lived experience of severe and persistent mental health issues and/or other disabilities
- Current Queensland driver's license.
- A Paid Blue card or the ability to obtain one is essential
- NDIS National Screening Check
- Rights to work in Australia
