About the role
The role is to manage a case load of clients under CHSP/HCP approved funding. Maintaining budgets, on boarding growth of new clients and managing care requirements to maintain clients independency.

* Time Management
* Understanding the Aged Care Standards
* Liaising with stakeholders and internal networks
What you bring
* Evidence of a minimum of 1 years experience in Case Management
* Relevant tertiary qualification in the community sector
* Demonstrated experience in the aged/community/disability sector particularly working with clients with complex needs
* Full Drivers Licence and own reliable registered and insured Vehicle
* Smart Phone
* Knowledge of My Aged Care portal/assessment process
* Willing to work in a team environment
* A desire to join a team committed to the care of others in a mission based organisation
* NDIS clearance (or willingness to obtain one)
* Police clearance (within the last 6 months Or willingness to obtain one)
