Work Hours: 38 hours per week | Monday to Friday | 8:00 AM – 8:00 PM
Rostered on-call availability required

We provide services and supports to adults, and families with accompanying children in the form of accommodation, case management, assertive outreach, financial assistance, community connection and referral services.
About The Role
We are seeking a committed and compassionate professional to join our team in Toowoomba, providing case management support within both community and residential settings. This role plays a key part in assisting individuals to achieve housing stability, greater social inclusion, and a sense of self-mastery that enables long-term tenancy and improved quality of life.

How You Will Make An Impact
In this role, you will:
* Ensure service delivery aligns with the National Homelessness Stream Model of Care, supporting a consistent, high-quality approach to homelessness services across the country.
* Building effective, supportive relationships with clients that promote self-determination and active participation
* Delivering tailored case management in line with The Salvation Army's models and best practices
* Conducting thorough assessments of needs, risks, and vulnerabilities
* Providing ongoing evaluation of client needs throughout the support journey—from intake through to exit
* Collaboratively developing individualised case plans that include practical interventions and strategies to overcome barriers
* Implementing and monitoring case plans in partnership with clients
What You Will Bring
* Tertiary qualifications in Social Work, Community Services, Welfare, or a related field – degree qualification preferred
* OR substantial sector knowledge and experience gained through professional roles and ongoing development
* Demonstrated experience in client assessment, case management, and the social services sector
* A current Working with Children Check (QLD Paid Blue Card)
* Current First Aid Certificate (if designated as a first aider)
* A valid QLD Driver's Licence
