Responsibilities:
* Conduct thorough assessments of client needs and develop personalized care plans.
* Collaborate with healthcare professionals to ensure comprehensive care delivery.
* Visit clients at home to provide emotional support and monitoring services.
Requirements:
* Relevant qualifications such as a Certificate IV in Ageing Support or Diploma in Nursing.
* Valid Victorian Driver's Licence.
* Working with Children Check.
