About the role
Grafton Community Health is seeking a Nurse Practitioner to orovide a high level of evidence based health care as a rural generalist - Aboriginal Health to improve access to care and address gaps in existing health care services through the provision of a flexible and innovative model of care based in the community setting.
Providing clinical and professional leadership as an expert in the clinical field and undertaking comprehensive health assessments as well as collaborating and consulting with other members of the health care team to plan, implement and evaluate care. This integrated approach will support best practice and maximise patient care for Aboriginal patients, families and communities.
