About the Role:
As Clinical Manager – Community Geriatrics, you will:
- Coordinate and manage community geriatric programs and outpatient services
- Provide clinical leadership and mentorship to nursing and allied health teams
- Ensure seamless transitions from hospital to home, supporting independence and wellbeing
- Champion best practice, quality improvement, and research initiatives
- Develop and implement evidence-based care plans
- Foster strong relationships with community partners and healthcare providers
- Ensure quality standards and best practice guidelines are maintained
To be eligible for this position, you must meet the minimum qualification requirements as aligned to either a Registered Nurse or Allied Health specialty, as defined in the relevant role description.
About You:
You are either a:
- Registered Nurse (RN3), or
- Allied Health Professionals (AHP3) in disciplines such as Physiotherapy, Occupational Therapy, Speech Pathology, Exercise Physiology, Dietetics, Podiatry, Psychology or Social Work.
with strong leadership experience and a passion for aged care.
You’ll bring:

- Proven ability to lead teams and manage resources effectively.
- Strong communication, problem-solving, and collaboration skills.
