This is a recruitment for Permanent / Fixed - Term, Full - Time appointment, with the possibility of extension and/or permanency.
Currently we are seeking a Clinical Nurse Specialist - MH Liaison GP to join our team. The successful applicant will be responsible for providing clinical leadership, consultation, and coordination to promote understanding between General Practitioners (GP's) and Rockingham Kwinana Mental Health Service (RKMHS) to ensure optimum access and delivery of quality patient care. You will also provide triage, assessment and time limited management and support within the areas of specialty and advanced practice to consumers with complex mental health needs, their families, and carers.
Rockingham & Peel Group (RPH) is part of the South Metropolitan Health Service, one of the fastest growing health services in Western Australia and comprises two hospital campuses, Rockingham General Hospital and Murray District Hospital, as well as community and mental health services across Peel, Rockingham and Kwinana. SMHS' Mental Health Services are delivered by two sites: Fiona Stanley Fremantle Hospital Mental Health (FSFH Mental Health) and the Peel and Rockingham Kwinana Mental Health (PaRK Mental Health), which delivers specialist mental health services across the South Metropolitan Health catchment area.
Responsibilities
• Provide clinical leadership, consultation and coordination between GP's and RKMHS.
• Triage, assess, and manage complex mental health consumers, their families, and carers in an advanced practice setting.
• Promote optimum access to quality patient care across community and hospital settings.
