About The Role
This role provides specialist therapeutic rehabilitation and case management services to adults and young people accessing the Mid North Coast Brain Injury Rehabilitation Service (MNCBIRS), with a strong focus on cognitive, behavioural and psychosocial rehabilitation. The position delivers advanced, person-centred clinical care to clients and their families/carers, supporting recovery, participation and quality of life following brain injury.
The clinician develops, implements and evaluates individualised rehabilitation plans and reports in line with icare, NDIS, CTP and WorkCover guidelines, while working collaboratively within multidisciplinary teams to deliver innovative, evidence-based rehabilitation models and therapeutic interventions. The role also involves close liaison with external funding bodies, agencies and service providers to ensure rehabilitation programs are flexible, coordinated and responsive to client goals.
Care is delivered within professional, organisational, legal and ethical frameworks, ensuring best practice and patient safety are central to all clinical activities. The role contributes to service development through participation in program evaluation, outcome measurement, quality improvement initiatives and research projects aligned with the NSW Brain Injury Rehabilitation Program (BIRP). Consumer participation and feedback are actively promoted to ensure services continue to meet the needs of clients and carers, while supporting a culture of clinical excellence, innovation and continuous improvement.
About You
You will need to hold a tertiary degree in Social Work that provides eligibility for membership of the Australian Association of Social Workers (AASW) and be able to bring demonstrated experience in the assessment and delivery of rehabilitation services within specialised settings, particularly in the areas of traumatic brain injury (TBI) and/or acquired brain injury (ABI).
You have proven experience in developing comprehensive rehabilitation plans and providing effective case management, supporting clients with complex needs across their recovery journey and are an approved icare/Lifetime Care Case Manager, or are willing to work towards this accreditation, and have a sound working knowledge of the NSW CTP, WorkCover and icare schemes.
You demonstrate excellent negotiation, decision-making and advocacy skills, with the ability to work collaboratively with a range of stakeholders to achieve positive client outcomes and you possess advanced reasoning skills and are able to apply independent professional judgement across a variety of complex clinical situations.
You have a strong understanding of person centred care, quality improvement and evidence-based practice, and you are committed to delivering high quality, client-focused services and you hold a current NSW Driver Licence and are able to travel as required.
