Ideal Candidate
The Registered Nurse is responsible for patient-centred, evidence-informed nursing care that promotes identified health outcomes. The Registered Nurse will assess, plan, implement and evaluate nursing care in collaboration with individuals and the multidisciplinary healthcare team. The role may involve working across multiple sites and within diverse models of care in the SLHD community mental health service. Shifts may occur across any of the community-based teams within the SLHD; shifts may be day shifts or part of a rotating roster depending on the Community Mental Health Team. There will be both community-based and centre-based duties in the role.

Responsibilities
* Provide patient-centred, evidence-informed nursing care to promote health outcomes.
* Assess, plan, implement and evaluate nursing care in collaboration with individuals and the multidisciplinary team.
* Work across multiple sites and within diverse models of care within the SLHD community mental health service.
* Contribute to prevention, early intervention, treatment and ongoing care within the community context.
* Cover shifts across community-based and centre-based teams as required.
Qualifications and Requirements
* Recognised tertiary nursing qualifications and registration with AHPRA.
* Demonstrated competencies to provide contemporary mental health practice.
* NSW Driver's Licence (P1 and P2 licences acceptable).
* Previous experience in Mental Health is desirable.
