About the role
The role is to manage a case load of clients under CHSP/HCP approved funding.
Maintaining budgets, on boarding growth of new clients and managing care requirements to maintain clients independency.

What you bring
Evidence of a minimum of 1 years experience in Case ManagementRelevant tertiary qualification in the community sectorDemonstrated experience in the aged/community/disability sector particularly working with clients with complex needsFull Drivers Licence and own reliable registered and insured VehicleSmart PhoneKnowledge of My Aged Care portal/assessment processWilling to work in a team environmentA desire to join a team committed to the care of others in a mission based organisationNDIS clearance (or willingness to obtain one)Police clearance (within the last 6 months Or willingness to obtain one)
