Key Responsibilities
* Provide holistic support in hub and in-home settings to enable individuals to reach their goals
* Interpret and respond to communication needs and health care plans to ensure seamless service delivery
* Administer prescribed medication and maintain accurate records to promote quality care
* Assist with mealtime guidelines and documentation of observations to foster a culture of continuous improvement
* Promote independence and community participation through person-centred planning and collaboration
* Contribute to care planning and collaboration with families, carers, and colleagues to ensure cohesive service delivery
Requirements
* Relevant qualifications and experience working with individuals with physical and intellectual disabilities
* Excellent communication and interpersonal skills to build strong relationships with clients, families, and colleagues
* Able to interpret and respond to communication needs and health care plans to ensure seamless service delivery
* Experience with medication administration and record-keeping to promote quality care
* Ability to work collaboratively as part of a dynamic team to achieve shared goals
