Join a unique, interdisciplinary community rehabilitation team where no two days are the same. Based in Toronto NSW, you'll deliver care across outpatient, home, community and telehealth settings – all while enjoying the lifestyle perks of Lake Macquarie and easy, free parking.

Westlakes Community Rehabilitation Team supports a diverse and clinically interesting patient group, including stroke, multi‐morbidity, frailty and falls, and neurodegenerative conditions.

You'll be part of a highly collaborative team of Physiotherapists, Occupational Therapists, Speech Pathologists, Social Workers, Nursing staff and Allied Health Assistants – all committed to strong teamwork, excellent communication, and truly patient‐centred, outcome‐focused care.

About The Role

This is a varied and rewarding role where you'll take the lead in running the outpatient rehabilitation gym across weekday mornings, supported by a skilled and supportive multidisciplinary team.

Following comprehensive, individualised assessments, you'll deliver tailored rehabilitation programs across multiple settings – including the outpatient gym, patients' homes, community environments, and via telehealth. Care is flexible and guided by patient goals, ensuring meaningful and relevant outcomes.

You'll work with patients in both group and one‐on‐one settings, supporting them to build strength, independence and confidence – and helping translate rehabilitation into real‐world function. This includes assisting patients to transition into sustainable, community‐based exercise programs.

You'll also play a key role in collecting and analysing outcome measures to support continuous quality improvement and enhance service delivery.

As part of an interdisciplinary team, you'll contribute to a range of shared responsibilities including intake, case management, care coordination, complex reporting, team administration and discharge planning.

About You

You're a motivated Physiotherapist who thrives in a collaborative, team‐focused environment and is passionate about delivering patient‐centred, outcome‐driven care.

You bring a broad rehabilitation skillset, a proactive approach to problem‐solving, and enjoy working with diverse and complex patient populations. You're curious, adaptable and committed to ongoing learning and professional growth.

In this role, you'll be supported to further develop specialist skills in areas such as behaviour change, falls prevention, subacute rehabilitation and neurological conditions.

Support

You'll be well supported by an experienced Team Leader and work Wednesday to Friday as part of a job share arrangement, collaborating with another Physiotherapist to ensure continuity of care.

Professional support is also provided by the Community Physiotherapy Head of Discipline, along with access to regular team meetings and professional development opportunities.

A structured orientation programme – including written resources, peer support and supervision – will ensure you feel confident and supported in your role from day one.

* Physiotherapist with current Authority to Practice with AHPRA
