Our clients are a mix of NDIS, Home Care Package and Private. Our office is located in Campbelltown.ABOUT THE ROLEConducting Client Care Assessments and Care PlanningManaging client compliance and a case load of clientsAnswer new client enquiries over the phone in a knowledgeable manner with the aim of scheduling a care consultationPerforming functions defined by established routines, methods, standards and procedures, with scope to exercise initiative in the application of new work practices and proceduresGaining a thorough understanding of all work processes and procedures relevant to their work areaAssisting the people we support in achieving planned, personal outcomes, which are clearly defined and regularly reviewedPerforming tasks of a sensitive nature including the collection and provision of personal informationAssisting with general administrative functions requiring sound judgment, initiative, confidentiality and sensitivityInitiating and maintaining a professional standard of communication with a broad range of individualsScheduling appointments and in-home services to appropriate staff based on discipline, availability and geographical requirementsCoordinating the provision of quality person-centred care according to the client's needs, service/care plans and available fundingGeneral administration with respect to tasks performed and recruitment of staffDevelopment of the business within the South West Sydney and Western SydneyJob Types: Permanent, CasualPay: $37.00 - $40.00 per hourExpected hours: 38 per weekSchedule:Day shiftMonday to FridaySupplementary Pay:Performance bonusApplication Question(s):Australian Driving License and Comprehensive car insuranceHave Completed Cert IV in Aged care or Disability Support or Health CareExperience:Home care: 3 years (Required)Work Authorisation:Australia (Required) Qualifications Cert IV in Aged care or Disability Support or Health Care
