**Program Summary**

The Malcolm's Youth Hostel is located in Kingaroy, Queensland and managed by South Burnett CTC (CTC). Malcolm's Youth Hostel has 6 client bedrooms, an office and a small self-contained bedroom for staff sleepover. Malcolm's Youth Hostel is funded to provide a high level of supervision in Immediate Supported Accommodation for 5 clients under Specialist Homelessness Service (SHS) Service Agreement with the support of trained Youth Workers.

Malcolm's Youth Hostel accepts young people aged 16 to 25 years old who are homeless. The aim of Malcolm's Youth Hostel is to work intensively with young people towards gaining safe, secure accommodation upon exiting the program. Each young person has an individual case plan to help them meet their individual circumstances and goals.

Case plans are developed by the clients and Youth Workers and uploaded to the database for all Youth Workers to action, revise and develop, with the client.

**Position Summary**

Youth Workers will assist each young person to build their skills to gain and sustain safe and secure accommodation.

The youth worker will provide support within Malcolm's Youth Hostel, with the youth worker having rostered shifts and being asked to fill shifts, sometimes at short notice.

**Attributes Required to Meet the Responsibilities of Position**
- Qualifications/Industry Experience/Licences_
- Qualifications in Youth Work, Social Work or Human Services or similar. Minimum requirement Certificate IV in Community Services, Youth Work, etc
- Two years or more of related industry experience which demonstrates the ability to engage with and support clients experiencing homelessness
- Demonstrated experience utilising trauma informed practice in engaging with and supporting clients towards positive outcomes
- Experience in supporting clients to successfully navigate and access systems to achieve and sustain housing
- Open Queensland Driver's Licence
- First Aid, CPR and Bronze Medallion Certificates or ability to acquire
- Prep-L Supervisor Course or ability to acquire
- Tenancy Skills: Skillsets for Successful Tenancies - Dollars and Sense or ability to acquire

- Time management and IT competency to ensure all assessments, case notes, training, and other responsibilities/deadlines are met and to ensure that client profiles are kept up to date within 24 hours on a database

- Availability and commitment to participate in a roster across days, nights, weekends, and public holidays

**Responsibilities of Position**

Broad outline of the responsibilities position includes:

- Assess referrals, contacting the young people, other key people, completing the associated risk assessment.
- Make a recommendation with risk mitigation strategies to the Hostel Coordinator and/or Housing & Homelessness Services Team Leader
- Welcome and induct new clients to Malcolm's Youth Hostel and familiarise them with its operation, rules, routine, general evacuation instructions, policies, and general expectations.
- Add the young person into the Queensland Homelessness Information Platform (QHIP), create an assessment and send a self-referral to South Burnett Youth Hostel in the QHIP database
- Support each young person to develop a case plan that is Specific, Measurable, Action Orientated, Realistic and Time bound
- Engage and support young people, on a daily basis, in residence in Malcolm's Youth Hostel
- Actively work with each young person, daily, to achieve and update their case plan and to build their case plan towards achievable, sustainable outcomes
- Support each young person to develop independent living skills which allow them to succeed in obtaining and keeping safe and secure accommodation. Provide support, coaching and prompting to embed these skills
- Directly support young people to identify and engage in groups, workshops, organisations, opportunities and/or volunteering to engage and connect with the community
- Reporting_
- Maintain accurate daily case notes as per the Case Notes Good Practice Guidelines located on the CTC Staff Intranet
- Create and maintain Details, Support Periods, Profiles, Accommodation, Status, Alerts, Consent, Referrals, Plans and Documents on the SRS Database.
- Record meaningful and accurate data in the Handover document throughout every shift
- Update client door passcodes when a client moves out, or on the first day of the month, whichever comes first
- Strictly follow the Hostel Medication Policy and Procedures
- Utilise internal and external stakeholder relationships to advocate for and achieve c
