POSITION DETAILS
Community Transport Service provides support to people to enable access and stay connected with their local community, resulting in increased social and economic participation and wellbeing. The service provides access to their community to gain or regain independence, support community participation, providing opportunities to enable access to other social and community services.
The purpose of the key support worker role within Community Transport is to work in collaboration within the team to implement and monitor Community Transport services by delivery and providing high-quality supports to clients. This service operates within the following regions: Darling Down East, Esk, Kilcoy, Lockyer Valley, Granite Belt and Toowoomba.
KEY RESPONSIBILITIES
* Complete Intake Screening and Service Bookings for Community Transport clients.
* Undertake and complete referral intake, assessment, and supported exit processes.
* Engaging in effective communication with clients to discuss their service needs, feedback, and enquiries.
* Liaising with the program manager to ensure that individuals are valued and respected and that support is person-centred.
* Outreach to local communities to ensure referral generation and information sharing on Community Transport services.
* Providing community Transport within the community and ensure WHS matters are adhered to.
* Communicate with program manager regarding client support needs/changes to services and any relevant updates.
* Assist with administrative functions as required, including maintenance of the client management system, checking clients service booking/client files, and monitoring the inbox for any service updates and enquires.
* Participate in any internal and external meetings as required in a collaborative and consultative manor.
* Actively participate in adopting a culture and language of hope and optimism
* Respect and encourage clients' independence, choice feedback and rights.
* Maintain the confidentiality of information regarding clients, carers and employees.
* Maintain a good understanding of client needs and overall well-being, including physical and mental health needs.
* Report any changes in the condition or needs of clients and/or hazards whilst transporting and during visits.
* Contribute to the smooth efficient and effective provision of services to clients by working as part of a team
* Carry out work in accordance with the Aged Care Quality Standards, the National Disability Insurance Scheme Practice Standards and/or the Human Services Quality Standards
* Liaise with the internal Community Transport team and Managers.

* Participate in supervision and ongoing learning and development opportunities.

KEY SELECTION CRITERIA
Essential
* Relevant Certificate in community services or significant progress towards such a qualification and demonstrated knowledge/experience of the provision of community-based support services which reflect respect, inclusion, equality, and a strengths-based approach.
* Demonstrate knowledge and or experience working with diverse client groups including people with a disability, and their families/carers, people with mental illness, people who are homeless or residing in marginalised housing and who may be at risk of homelessness.
* Demonstrate knowledge and experience in delivering community-based support.
* Ability to be respectful, calm, and encouraging towards individuals and have patience and understanding of others' recovery journey, including the ability to set boundaries and be self-aware.
* Ability to show initiative, think critically and make judgments based on sound reason and the information/evidence presented.
* Good written and verbal communication skills, and computer literacy.
* Current class C driver license including use of own private vehicle and insurance
* National NDIS Worker Screening Check or current Yellow Card
Desirable
* Current First Aid and CPR certificate
