Community Nursing Role
We are seeking a dedicated and compassionate Registered Nurse to join our multidisciplinary team in delivering high-quality person-centred care to improve the lives of seniors from culturally diverse backgrounds.
This pivotal role will work with our community aged care professionals to support older people by:
* delivering direct and comprehensive nursing care
* developing, implementing, and evaluating individualised care plans
* supporting patients with chronic disease management, palliative care, post-hospital discharge, wound care, and medication administration
* promoting health education and self-care among patients and their families
As a key member of our team, you will have the flexibility and time to concentrate on clients, often the most vulnerable members of society, and be able to build strong one-on-one relationships with people from all over the world.
Key Responsibilities
1. Deliver high-quality nursing care to older adults in community settings
2. Develop and implement individualised care plans to meet the unique needs of each patient
3. Work collaboratively with a multidisciplinary team to provide comprehensive support to patients and their families
4. Contribute to the development and implementation of health education programs to promote self-care and healthy living among patients and their families
Requirements
* Registered nurse with Australian Health Practitioner Regulation Agency (AHPRA) registration
* Experience working with client management systems and community nursing or aged care nursing
* Strong understanding of the complex health needs of older adults, including chronic disease management
* Passion for community nursing and person-centred care
