360 Health + Community is looking to welcome an inaugural Team Leader for our Peel Youth Mental Health Navigation and Coordination Service!
360 Health + Community is an established, experienced and growing not-for-profit organisation delivering specialised mental health services, headspace (x3), suicide prevention, psychosocial support, chronic health & aged care, allied health, Street Doctor, and NDIS support coordination and allied health services. We operate throughout the Perth metropolitan area, the Mid-West (out of Geraldton), and the Goldfields (out of Kalgoorlie). In delivering our NDIS support coordination and recovery coaching services we specialise in the psychosocial domain and work with complex participants, including those living with more than one disability.
Our youth participants (12-25 years of age) encompass a rich range of diversities – culture, language, religion, CaLD, Aboriginal and Torres Strait Islander, gender, sexuality, living circumstance, socio-economic status, educational and economic participation, and health status – and can include those with guardians, administrators, and child protection and family services.

About the role – Team Leader
The Peel Youth Mental Health Navigation and Coordination Service (PYMHNCS) is a new, innovative program and the inaugural Team Leader will play a significant role in implementing and then running the program. The Team Leader will lead a multi-disciplinary team of psycho-social service navigators supporting youth who are experiencing mental health conditions and biopsychosocial complexities. This program will work in concert with the Wandjoo Gateway, and a significant part of this role is ensuring collaboration and integration with the network to achieve improvements in the mental health care of youth.

Key aspects of this role:
- Building and leading a PYMHNCS team that delivers effective navigation and coordination of services for youth in the Peel and Serpentine/Jarrahdale area.
- Facilitating the design and use of recovery plans, based on the biopsychosocial needs of youth, which are trauma informed, culturally appropriate, and reduce the ‘re-telling’ burden on the participant.
- Contributing to the effectiveness and improvement of the Wandjoo Gateway service model by modelling best practices, advocating for and collaborating on change and improvement opportunities, and incorporating the views and feedback form youth using the service.
- Enhancing both the youth participants' development of their own agency and the Wandjoo Gateway service providers responsiveness to the biopsychosocial needs of youth.
What we are looking for / being successful in the role, you will have:
- Tertiary qualification (Social Work, Psychology, Occupational Therapy, or Nursing) – with corresponding Mental Health Accreditation.
- Minimum 5 years’ experience in the mental health or community service sector(s) working with clients who have complex needs.
- Experience in biopsychosocial assessment and recovery planning and the identification and assessment of risk working with clients having complex needs.
- Proven ability to lead and develop a multi-disciplinary team working with youth.
- Strong stakeholder network development and management skills.
- Sound verbal, written communication, and negotiation skills.
- A current full drivers' license (essential), access to a roadworthy and comprehensively insured motor vehicle, National Police Clearance and Working with Children Check.
360 Health + Community offers:
An attractive remuneration package based on:
- $53.89 to $62.53 per hour PLUS superannuation.
Starting rate based on the qualifications and experience you bring to the role. Appointments are usually made at the starting rate for the band.
