You'll be the go-to person for crafting creative, personalised care plans that really make a difference.

- Permanent full-time role - Company pool car available or be reimbursed at $0.96 per km when own vehicle used.
What You'll Do: Build real connections with awesome clients Turn chats into care plans that actually make a difference Help clients get the most out of their budgets Mix it up, some days on the road, some at home in your comfies Team up with health pros to get the best outcomes Spread the word and help our services reach more people What You'll Bring: Minimum Certificate III in Case Management, Aged Care, Community Services or Disability Services Previous experience in case management within aged care, disability, or youth services highly preferred Experience helping people through case management A way with words, and people!
