Your Key Responsibilities
Direct Client Support
* Engage clients in meaningful community activities and social connections
* Provide assistance with domestic tasks including light cleaning, cooking, and meal preparation
* Offer safe, reliable transport to appointments and activities
* Facilitate uplifting, trauma-informed conversations that promote healing and growth
Professional Documentation & Collaboration
* Maintain accurate, detailed case notes and progress documentation
* Prepare incident reports when required
* Collect and implement data for allied health professionals
* Follow individualised Case Management and Behavioural Support Plans
* Collaborate effectively within our multidisciplinary team including clinical staff, house managers, families, and guardians
Goal Achievement & Advocacy
* Support participants in working toward their NDIS goals
* Advocate for client rights, preferences, and autonomy
* Contribute to positive outcomes through consistent, person-centred care
