About the Role
This position involves providing high-quality client-focused care in line with tailored care plans.
Key Responsibilities
* Support clients with daily living activities as per individualised care plans.
* Work collaboratively with registered nurses to deliver client-specific care including personal hygiene, nutrition and mobility.
* Identify client care needs and ensure holistic care through collaboration with the client's care team.
* Communicate effectively with clients, families and team members.
* Assist clients with light housekeeping duties promoting independence, including laundry, cleaning and meal preparation.
