About the Role: As a Community Care Worker, you will be responsible for delivering person-centred services across aged care, disability and community service settings.
Your Key Responsibilities:
* Empower individuals to live more independently, safely, and meaningfully by providing personal care (showering/dressing), continence support, assistance with meals, assistance with medications, domestic duties (laundry/housework/ironing/gardening).
* Taking clients shopping providing transport, in home respite, centre-based respite care, overnight respite (including sleepovers), social support/activities.
* You will provide the ability for clients to access other relevant services, and any other tasks as stated in the customer care plan.

* The ability to work as part of a team to continually promote and provide excellent care and services to customers.
* Demonstrated ability to provide excellent, flexible and holistic care to a variety of customers with varying needs.
* Demonstrated understanding of the needs of and empathy towards older people and people with a disability.
* Willingness to travel in the community and between clients for work.
* Certificate III in Individual Care (or equivalent) from a recognised provider for appointment to Level 3.
* Comprehensively insured Motor Vehicle, Current Queensland Driver's Licence and willingness to drive and use own vehicle.
