Job Summary
* We are seeking a dedicated professional to join our adult rehabilitation team as a Social Rehabilitation Specialist.
The Role:
* You will be responsible for conducting thorough assessments, planning and delivering effective social work interventions for clients with complex care needs.
* In collaboration with the interdisciplinary team, you will provide comprehensive care that addresses the unique needs of each client.
* You will accurately maintain records, case notes and discharge summaries to meet the required professional standards.
* Possessing strong clinical skills, you will engage in regular supervision and commit to ongoing professional development to stay current with best practices.
The CRU Service and Team:
* We deliver an outpatient rehabilitation service working with clients who have diverse diagnostic conditions, primarily acquired neurological conditions and progressive neurological conditions.
* Ours is an interdisciplinary team comprising Occupational Therapy, Physiotherapy, Psychology, Dietetics, Social Work, Rehabilitation Nursing, Art Tutor, Allied Health Assistants, all focused on client-centered goal setting and rehabilitation planning.
Requirements
* You will require knowledge of contemporary social work and client-centered practice, including the clinical management of clients with complex care needs relevant to an adult rehabilitation context.
* Proven ability to exercise professional judgment, take initiative, and work with general supervision while seeking guidance as needed.
