We are currently seeking a dedicated professional to join our team as a Social Worker/Specialist Support Coordinator. The successful candidate will work closely with clients, their families, and healthcare providers to deliver high-quality support services.
The role involves working with individuals who have complex care and medical needs, across various age groups, to identify and address barriers to implementing their NDIS plan. Strong communication and interpersonal skills are essential for building relationships and collaborating with stakeholders.
To be successful in this position, you will require:
* A bachelor's degree in social work or a related field (Occupational Therapy, Physiotherapy, Psychology, Nursing, or Allied Health);
* Well-developed oral, written, and interpersonal communication skills;
* Able to work independently and collaboratively as part of a team;
* Established ability to listen actively and demonstrate empathy;
* Organisational skills, with the ability to manage time effectively and prioritise tasks;
* Current Working With Children card; NDIS Worker Screening; Police check (within 6 months); current Drivers Licence and roadworthy vehicle;
* Computer literacy – efficiency in Microsoft Office;
* Experience working with clients with dual diagnoses, preferably in Forensic Disability or Forensic Mental Health;
* A willingness and ability to travel by car, 4WD, and/or light aircraft to rural and remote settings;
* Experience working with Aboriginal and Torres Strait Islander peoples, and an interest in indigenous advancement;
