About Us:
The Nurse Navigator Service is part of a state-wide model of nursing care designed to provide holistic support to patients with chronic and complex conditions requiring an expert level of healthcare support to improve patient outcomes and quality of life. This integrated and proactive approach to health care service delivery will help ensure the most disadvantaged, vulnerable and HHS intense patients receive the most appropriate service when and where they need it throughout the continuum of care.
About the role:
* This position requires effective computer skills or the ability to acquire computer skills relevant to the requirements of the position in a short time frame e.g., Telehealth, Patient Flow Manager, HBCIS, The Viewer, integrated Electronic Medical Record application (ieMR), Office, CHAI. Collection of Nurse Navigator activity, referral management and the input to Clinical Administrative Systems is a requirement of the role.
* Facilitate effective patient centred care by enhancing systems integration, reducing fragmentation and barriers, and establishing a personalised care pathway – for example, using Telehealth or other technology to facilitate care coordination.
* Provide strong leadership to motivate the patient and multidisciplinary team to achieve agreed care pathway planned goals within designated timeframes.
* Establish therapeutic relationships with patients founded upon sensitivity to cultural, linguistic, and other differences, and respect for personal lifestyle choices which promote self-management and seek to improve patient health literacy.
