On 1 January 2026 five health services (Kooweerup Regional Health Service, Peninsula Health, Alfred Health, Bass Coast Health and Gippsland Southern Health Service) will come together to create Bayside Health. A newly merged health service with more than 22,000 employees caring for a community of over 1.2 million Victorians.
The new health service will provide people with the best possible care with services that support them throughout their lives. The merger will also strengthen our workforce, providing new training, education and career opportunities. Subject to final approval on 1 January 2026, all employees from the five health services will transfer to Bayside Health.

What You Will Be Doing
The Peninsula Health Homelessness and Health Outreach Service supports vulnerable clients who are at risk of homelessness with housing support and service system navigation.
The Case Manager role provides specialist and intensive case management to help clients understand and access housing options and aged care services, and connect with other relevant supports in their community. The program provides case management via assertive outreach to assist older people who face barriers to accessing housing and services and are vulnerable, marginalised, and disadvantaged.
The role is required to provide holistic support and pathways to address their client's wellbeing and health needs. For example, health concerns, mental health, AOD, income supports and material aid. Some brokerage funds are available to support clients with housing establishment and other crisis needs.
What You Need
We anticipate you will have:
Degree in Social work, a Diploma in Community Services, Bachelor of Nursing (RN Div 1), Diploma of Nursing and hold a relevant registration with the required governing body.
Experience working in health care, community health or outreach settings.
Experience in case management or similar modalities.
