About the Job
We are seeking an experienced physiotherapist to join our team in providing care to clients under the Transitional Aged Care Program.
* To provide physiotherapy assessment and treatment for clients, maximizing functional improvement through slow-stream interventions at home.
* To contribute to client outcomes through case management, care planning and ongoing evaluation of service needs within a community-based model of care.
* To deliver clinical supervision and support to junior physiotherapists and allied health assistants for safe practice and professional development.

*Please note that you must meet all requirements as stated below*,
• Current registration with Australian Health Practitioners Regulation Agency (AHPRA) as a physiotherapist
• Current NSW Class C or above driver's licence.
• Experience in advanced assessment, clinical reasoning and evidence-based intervention in aged care or community rehabilitation setting.],
