What You'll Be Doing
We are seeking a dedicated and compassionate Social Worker to join our team on a permanent part-time basis at Tomaree Community Hospital. In this rewarding role, you will deliver professional social work services across both inpatient and Emergency Department (ED) settings, providing essential support to patients and their families during times of need.

Your work will involve comprehensive psychosocial assessments, complex discharge planning, counselling, advocacy, and referrals to appropriate community and support services. You will collaborate closely with our multidisciplinary team to ensure that care is holistic, coordinated, and responsive to each individual's unique circumstances.

About You
We are looking for an experienced and motivated Social Worker who is committed to delivering high-quality, patient-centred care. You will bring strong communication and teamwork skills, enabling you to engage respectfully and effectively with patients, families, and colleagues across all areas of the hospital. Your proactive approach to problem-solving and advocacy will support complex discharge planning and help ensure positive, well-coordinated patient outcomes. A commitment to ongoing professional development, ethical practice, and the values of NSW Health will be essential to your success in this role.

Requirements

* Bachelor in Social Work or equivalent with eligibility for membership with Australian Association of Social Work (AASW)
