As a skilled healthcare professional, you will deliver high-quality patient care in our residential aged care facility. Your role involves working closely with the Aged Care Service Manager, Assistant Nurse Unit Managers, allied health professionals, medical staff, families, and residents to provide person-centered care tailored to individual needs.
Duties:
* Establish meaningful relationships with residents to achieve optimal outcomes.
* Support resident care based on their best interests.
* Apply your knowledge and skills to conduct thorough assessments, develop, and document care plans.
* Implement care plans by providing direct care to residents.
* Evaluate the effectiveness of care through critical thinking and reflective practice.
* Coordinate discharge planning processes for residents in short-term respite arrangements.
In this role, you will be responsible for maintaining standards in accordance with the Aged Care Quality and Safety Commission and Aged Care Standards.
